Default HTML

Does anyone know how to make this work?
I added the second drop down menu through coding but it one register in the check out line it only picks up the one drop down box and the treats that are in that

I would hate to have to make my customer type in the two treats that they want, but i cant fund any other way to do it...or leave out the toppings please help

yeah, i copied the current drop down menu and pasted it into the...html thing where i posted the whole html code from paypal i just dont know what to do or what part to paste to make it show up in the shopping cart too!!

I can use to text boxes aned have the customer put there two treat choices in one text box and toppings in the other text box but i just feel like it would be a pain for the customer and not so preofessional
